DELTA DENTAL INSURANCE DHMO General Information
The HP Retirees Club in the Bay Area has negotiated an insurance plan (DeltaCare
USA - DHMO) with Delta Dental for their current members residing in California. They
have extended this offer to our Club SoCal members as well. Those who wish to take
advantage of this must maintain membership in our club and be living in California.
Neither HP nor the HP Retiree Clubs have any responsibilities or liabilities
regarding this insurance. This plan is NOT administered by the HP or the clubs;
it is administered by Delta Dental of California.
To enroll, send an email to Joan Hensley at [email protected]. She will send you the enrollment form and instructions directly.
Once you are signed up, all interactions will be between you and Delta Dental. Note
that plan this is a DHMO so you will need to use professionals from within the plan. Not
all dentists who accept Delta Dental Insurance accept DeltaCare USA plans, so be sure
to confirm that you are satisfied with the selection of dentists before signing up.
Costs
The effective dates: January 1, 2027 - December 31, 2028
DHMO 2027-28 Quarterly Rates
Enrollee $99.63
Enrollee + 1 dependent $157.35
Enrollee + 2 or more dependents $226.38
You can view the latest Combined Evidence of Coverage and Disclosure Form (56
pages) and Highlight Sheet (16 pages). PDF documents are provided by Delta Dental.
You may enroll in the plan at any time. Delta Dental usually takes about a month to
process your enrollment, so you should receive your membership card by the first of
the FOLLOWING month. Your dentist's office will be notified of your enrollment during
this period.
